your name and address as it should appear on the official tax receipt

name of the endowment fund to which you are giving (If a fund name is not provided, your gift will be added to the Foundation’s
Community Building Fund)
a phone number or email address where you can be contacted if there is a problem processing your gift.
if you are giving to multiple funds, the specific amount to be allocated to each fund

Automatic credit card or direct bank withdrawals
Regular withdrawals can be set up on a monthly, quarterly or annual basis to any endowment fund. If you are interested in arranging for automatic credit card or direct bank withdrawals for donations to any endowment fund, please contact our Donor Relations staff at 944.9474.
